49. When used for conscious sedation, nitrous oxide may
1. produce signs of inherent myocardial depression.
2. produce an indirect sympathomimetic action..can anyone explain exactely what does this mean????
3. cause the patient to sweat.
4. produce numbness of the extremities.

A. (1) (2) (3)
B. (1) and (3)
C. (2) and (4)
D. (4) only
E. All of the above....correct

can anyone explain 2 and 1 for me?
I know that can cause numbness if the use is chronic...but I don't know if they are asking only about ocasional use

Indirect sympathomimetics cause norepinephrine released from presynaptic terminals, but do not bind to adrenergic receptors. These drugs enter the presynaptic terminal and displace stores of norepinephrine from storage vescicles. They potentiate the effects of norepinephrine produced endogenously but they dont directly affect postsynaptic receptors.
Whereas the direct-acting bind to adrenergic receptors without interacting with the presynaptic neuron. The activated receptor initiates synthesis of second messengers and subsequent intracellular signals.

CARDIOVASCULAR EFFECTS
Nitrous oxide does not produced any clinically significant cardiovascular effects. It has a weak and dose-dependent myocardial depressant effect as well as a mild sympathomimetic effect. These opposing influences tend to cancel each other, leading to no change in cardiac output.

Im not sure about the last choice because when used it for conscious sedation, nitrous oxide usually provides only a tingling sensation of the hands and feet including other possible symptoms such as a feeling of relaxation, body warmth, circum-oral numbness, auditory effects and euphoria. As the dose increases, the patient is more likely to develop adverse symptoms such as dysphoria and nausea.

I hope this helps...

. Which of the following muscles has two separate functions in mandibular movement?
A. Masseter.
B. Geniohyoid.
C. External (lateral) pterygoid.
D. Buccinator.

The university of minesota...has this questions in their review..but they say the masseter is the right answer????

Can anyone explain?????
is it masseter or lateral pterygoid?????

If I will be the one to answer this question I will choose both lateral pterygoid and masseter because of the ff. reasons:
First reason:
As we know that lateral pterygoid has two bellies (a. inferior, and b. superior.) which function quite differently.
When the right and left Inferior lateral pterygoid contract simultaneously, the condyles are pulled down the articular eminences and the mandible is protruded. Unilateral contraction creates a mediotrusive mov't. of the condyle and causes lateral mov't. of the mandible to the opposite side. When this muscle functions with the mandibular depressors, the mandible is lowered and the condyles glide forward and downward on the articular eminences.

Whereas the superior lateral pterygoid is attached to the anterior border of the articular disc. When this muscle is active, the fibers that are attached to the disc pull anteriorly, therefore the superior lateral pterygoid is technically a protractor of the disc ( However, the clinician should remember that this muscle is also attached to the neck of the condyle. This dual attachment does not allow the muscle to pull the disc through the discal space. However, protraction of the disc does not happen during jaw opening. When the inferior lateral pterygoid is protracting the condyle forward, the superior lateral pterygoid is inactive and therefore thus not bring the disc forward with the condyle. The superior lateral pterygoid is activated only in conjunction with activity of the elevator muscles during mandibular closure or a power stroke {Mg't of TM Disorder & Occlusion by J.P. Okeson}). In short lateral pterygoid helps in both protracting and depressing the mandible.

Second reason:
On the other hands we also know that masseter is made up of two portions or heads:
a. the superficial portion
b. the deep portion
As the fibers of the masseter contract, the mandible is elevated and the teeth are brought into contact. The masseter is a powerful muscle that provides the force necessary to chew efficiently. Its superficial portion may also aid in the protruding the mandible. When the mandible is protruded and biting force is applied, the fibers of the deep portion stabilized the condyle against the articular eminence.

Since you were given four choices But have to pick only one. Its up to you to choose the best one ! Goodluck !

. After initial setting, a chemically cured glass ionomer cement restoration should have a coating agent applied to
A. hasten the final set.
B. protect the cement from moisture...correct ?????
C. ****** the final set.
D. protect the cement from ultraviolet light.
E. create a smooth finish.

I know we should put a coating agent to seal..but they don't give that as an option

Isnt it that when you apply the coating agent to a chemically-cured glass ionomer you are sealing it against the oral fluids to protect it esp. from moisture contamination?
GIC is very sensitive to moisture contamination. If exposed to external moisture during setting reaction , the reaction is interrupted, resulting in a cement with high solubility and poor physical properties.

which of the foll modifications to the standard procedure for mixing gypsum products will increase the compressive strength of set material
\

adding small amt of salt to the water before mixing
decreasing the water /powder ratio by a small amount
using warmer water \
decreasing the mixing time

adding small amount of salt to the water before mixing can upset the delicate balance of the gypsum properties but can be used to control setting time and setting expansion.
using warmer water has nothing to do with its compressive strength but affects setting time.
decreasing mixing time decreases its compressive strength, in general, with an increase in mixing time, the strength will be increased to a limit that is approximately equivalent to that of a normal hand mixing for 1 minute.

C. orbital process of the zygomatic bone.
D. zygoma and the zygomatic process of the maxilla. ...correct

I am confused about this one..if anyone can explain

The zygomatic process of the maxilla is an extension of the maxillary surface that arises in the region of the apices of the first and second molars and serves as the articulation for the zygomatic bone.
So the prominence of the molar apices will be superimposed by these bones during the bisecting angle technique.

sequestration in body fat because the short acting barbiturates metabolizes by redistribution to the tissues of the body such as fat

If an opposing tooth intrudes severely into the edentulous space, it is not enough just to replace the missing tooth. To restore the mouth to complete function, free of interferences, it is often necessary to restore the tooth opposing the edentulous space. In severe cases, this may necessitates the devitalization of the supraerupted opposing tooth to permit enough shortening to correct plane of occlusion. BUT considering the age of the patient (Tooth preparations must account for the structure of the dental pulp chamber. Pulp size, which can be evaluated on a radiograph, decreases with age 50, it decreases more occlusocervically than faciolingually), since the patient is 60 years old the pulp size of tooth 1.6 might have already been reduced its size that may permit enough reduction without devitalizing it.
The preparation for full metal crown requires about 1.5 mm clearance on the functional cusps and 1.0 mm on the nonfunctional cusps.
Extrusion of the opposing tooth of more than 4 mm into the edentulous space should be extracted.
Hope this help! Goodluck on your best choice answer :) .

The American Academy of Pediatric Dentistry defines dental neglect as failure of the parent or guardian to seek treatment for caries, oral infection, or oral pain, or failure of the parent or guardian to follow through with treatment once he or she is informed that the aforementioned conditions exist. Thus the guidelines of dental neglect are: Untreated rampant caries
Untreated pain, infection, bleeding, or trauma
Lack of continuity of care once informed that above conditions exist
