Observership for international medical students

Hello everybody.
I am an Italian medical student in my 4th year in a 6 years degree. I know I'm not eligible for clerkship/clinical elective because I'm in not in the final year and I have no USMLE step 1, but I am looking for a summer observership/internship (is there any difference among observership/internship/clerkship?) in IM or cardiology.
I'm planning to send some emails with my CV to doctors of the hospitals I'm interested in. Do you think that in this way I could find a spot? Any hospital International students friendly to suggest?
