Anybody else having trouble collecting for post op pain regional blocks? We are having more and more trouble.

No answer as far as billing as we don't do them much in our facility. Only once the patients have exhausted all modalities in the PACU.

Simple answer, stop offering the service unless absolutely necessary.

No problems. Might want to start by having the surgeon document the request/need for the procedure somewhere (in note or preop orders).

We have switched to epic and there is a big box that always gets checked stating the block was done at his highness's request (since apparently we lack the clinical judgement). Another slight. Par for the course.

We treat our blocks like any other pain consult. Our block form has a blank line on top naming the doctor requesting the pain consult. Either the surgeon's or primary's name always written on that line as well as the reason for the pain consult.

I don't see it this way. Surgeons get a bundled payment that includes post-op care (including post-op pain management). Now that a second party is asking to get paid for post-op pain management, I think its reasonable for payers to ask that the surgeon actually wanted this.
