We treat our blocks like any other pain consult. Our block form has a blank line on top naming the doctor requesting the pain consult. Either the surgeon's or primary's name always written on that line as well as the reason for the pain consult.

I don't see it this way. Surgeons get a bundled payment that includes post-op care (including post-op pain management). Now that a second party is asking to get paid for post-op pain management, I think its reasonable for payers to ask that the surgeon actually wanted this.
