Here are some articles/book chapters from the Oral-Maxillofacial Surgery perspective.

There are also a whole ton of dental appliances also in use, but those still generally remain in the realm of the general dentist. They generally generate mixed results. Not bad to try as an additional therapy along with CPAP...

But when all else fails, there is always the good old Bi-Maxillary Orthognathic Surgery Advancement with very high success rate.
