Still stuck at 4 interviews ( all in the first week and now silence)
i have a feeling that if you were not called in the initial cycle for the ones on the list might as well cross them out because a rejection is imminent

To counter that, I actually just received an interview with Wake Forrest today and they had been on this list for quite some time before I got the invite. So that is good news for all of us still hoping to hear from schools that have apparently sent out interviews already, they are still sending out more. Good luck to everyone

I think there is some confusion here what institute is UAB. I think the OP thought it was University at Buffalo, which it is not :)

Univ at Buffalo is SUNY buffalo and it's a very good program. More research oriented than Einstein (Philly). And I agree with you that it's better than Einstein. But I think it depends on what you are looking for. Heard Einstein (Philly) is a good clinical program.

Any thoughts on the Texas Tech Pulm/CC program? Interviewing there later this year...from those I've talked with, they say good things about the IM residency but haven't heard much about the fellowship (possibly because it's just a couple years old). Anyone have any experience with or heard anything about them?

Another very quiet day. Heard AGH (Allegheny General Hospital) sent out some rejections though. Not sure about invites.

It's a clinical program, not a research program. If that's what you want the clinical exposure is great, they do have a varied level of pathology with more than a few zebras running around. The program takes 3/year but they want to expand it to 4/year.

The hospital itself is in big financial trouble and they've basically put all their eggs in the let's hope highmark gives us lots of money, which seems to be happening, but that's not a done deal yet, rumor is September, but frankly I think it makes more sense for highmark to let them go bankrupt, then take over.

MICU isnt the newest or nicest but it's functional, as fellow you're the only fellow for 2 teams so you come in, check to make sure no one is actively dying, adjust the vents and get weaning started. You supervise every admission with residents and end up doing lots of procedures. The medicine residents tend to be ambivalent about them, and the IM program has made it exceedingly hard for them to become signed off on lines so frankly I just do most of the lines myself given how busy it can be its easier to spend 15 minutes myself to do it versus an hour with a resident. The MICU isn't very forward thinking, ie support services are minima,, I've never seen a PT in the unit, much less walking a tubed pt. agh does Unfortunatly have a very high rate of end stage cancer pts who end p in the unit without any semblance of a code statues addressed so there is a lot of futile cancer care. Other ICUs we rotate in are CCU, TICU, NICU, SICU.

They're in the process of doing all of their own airways instead of some cocky CRNA who cowboys up and does things half-assed and frankly scarily. For the moment they do not have they're own glide scope, so back up is a boogie or fiberoptic for the moment.

Other procedures you do, chest tubes, lines including temp dialysis catheters, a lines, LP, thora, paras, swans (but they're very rare),

Crics are done by TICU attendings in the rare case you need one, I've had to have 3 done.

call is a float system for the time being its divided in mon-thurs nights & fri-sun nights and someone to cover sat/sun days.

Consults are busy with lots of basic copd and cancer, but there are good cases out there. Even on consults you do a lot of critical care for CCU and the step down unit.

They run the county health TB clinic, so you'll be very familiar with treating LTBI and you will have more than a few active cases through your time there, due to this, you will also be exposed to several MOTTS and they've written up several new MOTTS causing disease.

Clinic isn't a true fellows clinic, you have an assigned preceptor that you go to clinic with, and it highly depends on your attending on how that is run.

The lab is nothing fancy, and up until last month used primarily fiber optic scopes, they just upgraded to have more than one video scope. They have linear EBUS, Super-d nav. You will do some brachytherapy catheters. There is essentially no stenting, lazer is very rare, and they've been pushing to do more fudicials. They do a lot of PFTs, and you will be exposed to sniff tests, Methocholines, they just got mannitol provocation testing, and CPETs.

There is a new US for the MICU with chest and vascular probes, and the lab has an old sonosite for the floor as we put in lots of u/s guided pigtail chest tubes.

There is no transplant (thank god) as their program was shut down, it's hard to start a program when you're located in between UPMC and CCF.

No CF, but due to new Standards they're working on getting us in with UPMCs CF clinic. And they still haven't decided how they're going to comply with the new mandate for OB-critical care.

Pulm htn is managed by cardiology.

lectures include X-ray conference, board review, a weekly case conference, an attending lecture each week, and every week there is a multi-disciplinary conference that is well done.

The cafeteria is awful, there is a coffee cart that is good,

The EMR is awful (sunrise) but they keep saying they're upgrading to EPIC. That'll be done by 2018 I'm sure.​

You did an anesthesia residency prior to an IM residency? What made you decide to go the IM route and PCC instead of the CC care route through anesthesia? Why did you leave gas?
