Hi just wondering if anyone can help me clarify/confirm some questions regarding NJ MPJE.
1.Can you write multiple medications on one prescription blank?
2. Labeling- there is a need for expiration date?
3. Also when I was reading regarding more than 30 supply of a CII, they can give a 90 supply but total 3 diff prescriptions in advance? But it still means only 30 days supply can be dispensed at one time? Is there a certain documentation for that?
4. Also regarding changes for control II substance prescription, Is it ok to ADD a strength, quantity, directions etc if it is missing by calling MD or is it you can only CHANGE?
5. Transfer of rx for controlled substances II-IV, I see that is on one time basis, this is meaning you are able to transfer all remaining refills or just 1 time refill?
6. Can you fill an out of state controlled substance?

It is not required that only one drug be prescribed per blank. Note: Current law requires no more than one “scheduled” drug per blank (the word scheduled means a controlled substance, CII-CV).

A separate NJPB shall be utilized for each prescription written for a controlled dangerous substance.
The licensed prescriber's Federal DEA registration number shall be pre‐printed, legibly written,
typed, stamped, or otherwise affixed to the NJPB. No other medication shall appear on the
prescription.

If a licensed prescriber utilizes an NJPB pre‐printed with multiple drugs, the prescriber shall
obliterate, by a cross‐off procedure, any drug that is not being prescribed.
2. Labeling- there is a need for expiration date?

N.J.A.C. 13:39-11.10 Information Required to Appear on Prescription Label

a). The dispensed container for any sterile admixture product shall bear a permanently affixed label with at least the following information:

1. Date and time prepared;

2. In the retail pharmacy only, Prescriber name;

3. Patient name;

4. Directions for use;

5. Name and quantity of all active ingredients;

6. Name, address, and telephone number of pharmacy;

7. Use by date and time of sterile preparations (If no time is stated, it is presumed to be 11:59 P.M. of the stated

expiration date;

8. Any ancillary and cautionary instructions as needed;

9. As pertinent, a warning consistent with applicable Federal and state law that cytotoxic products are bio-hazardous; and

10. As pertinent, requirements for proper storage.

Applicable to CIIs only: May issue multiple prescriptions for a 90 day supply. Each of 3 prescriptions must be for a legitimate medical purpose, must indicate on other than the first prescription the earliest date it may be filled and it must not create an undue risk of diversion. (Also valid in NJ. See N.J.S.A. 5:9-22.9 and N.J.A.C. 8:65-7.5.

Also: If written no quantity limitation, can dispense “required amount”. See N.J. rules which only allow 30 days or 120 dosage units, whichever is less.

g. The following items cannot be changed orally: date, name of the controlled substance, strength, dosage form and quantity (NJ Board of Pharmacy News August 2010).

You cannot transfer a CII!
The transfer of original prescription information for CIII or IV (no CII) for the purpose of refill dispensing is permissible between pharmacies on a one time basis subject to the following:

1. The transfer is communicated directly between two licensed pharmacists and the transferring pharmacists records the following information:

i. “VOID” is placed on the face of the prescription;

ii. On the reverse side of the invalidated prescription the name, address and DEA number of the transferee pharmacy and the name of the pharmacist receiving the information from the transferee pharmacy;

iii. The date of the transfer and the transferor pharmacist;

The pharmacist receiving the transferred prescription information shall reduce to writing the following:

i. “TRANSFER” is written on the face of the prescription

ii. All information required pursuant to N.J.S.A. 24:21-17 above is provided along with the following:

(1) Date of the original prescription;

(2) Original number of refills authorized on the original prescription;

(3) Date of original dispensing;

(4) Number of valid refills remaining and the date of the last refill;

(5) Pharmacy’s name, address and DEA registration number and original number from which the prescription information was transferred;

(6) Name of the transferor pharmacist;

The original and transferred prescription must be maintained for a period of two years from the date of the last refill.
6. Can you fill an out of state controlled substance?

Yes, if the presenting patient and/or prescribing practitioner is known to the pharmacy and there is no reason to question the prescription's authenticity.​
