radioresistant splenomegaly !!!

Dear Colleagues,
I need your opinion concerning my 60 yo patient having a long date myeloproliferative disorder. she has huge splenomegaly 24cm sup-inf lengh, 2550 cm3 estimated volume, clinically symptomatic (pain, pancytopenia).
she was referred to RT. she was planned for daily low dose sessions of 0,5 Gy. there was no shrinkage after 6GYy. so we decided to continue till 20Gy.
now she is at 16 Gy and still no response.
she is already taking jakavi.

How frequent do you see that ? is there any other suggestions for RT? what could be the explanation other than fibrosis? thank you !

Personally I would have stopped treating at a far lower dose.
This is either fibrosis or infiltration by some hematologic malignancy (lymphoma?). It's not solely extramedullar hematopoiesis.
