So as I understand it , there is no way for programs to know if you are a reapplicant?? Esepcially if you take a research year and delay your graduation year to the next. ( of course they will see you have been in medical school for 5 years, but pretty common these days)

Of course for any programs that offer you a re interview the second time, probably good to mention it for full disclosure.

There is no way to know you're a reapplicant, regardless of whether you extend graduation or find a gap year option, other than looking at last year's list of applicants. If you're applying to new programs / new fields, then there is no way to know at all.
