Rural concierge?

Is it possible to run a rural concierge practice with a cheaper subscription/larger panel? The idea being to manage things via email, the clinic under your home, or the helipad in the backyard lol. I think it would be legit to do a dual FM/EM program, live in an underserved area, and basically do anything that isn't surgery. Could you afford things like in house xray, basic labs, and maybe even CT? Just dreaming here. Don't crush my dreams.
