Need some help for in office billing of SCS Trial

I had an Aetna patient and did an in office trial. We got the auth to do the 63650 x 2 and the L8680 x16 per the auth dept from Boston

Does anyone know how to bill this

This is the advice I got from the rep but wanted to get some more feedback

63650 1U
63650 1U Modifier 51
L8680 8 Units
L8680 8 Units Modifier 76


Thought or advice is greatly appreciated, first trial since fellowship and hoping to get paid something for it

If you got the auth for it, then pretty much as you listed it, I think: "63650 x 2 and the L8680 x16". Plus the two programming codes

Medicare on the other hand is only 63650 (and programming I think)
 
how many of you charge the programming codes?

what my rep said was that you needed a lot of extra documentation for that, and could get dinged for fraud
