I am currently a 3rd year student. My main goal is to work with athletes and be the guy on the sidelines at football games, ect. What would be my best choice of specialties with that goal in mind? I know ortho but beyond that?

Sports Medicine fellowship at VCOM http://www.vcom.edu/sportsmed/ You'd be on the sidelines at Virginia Tech. Does it get better for someone with your goals in mind? I think not.
 
So far the options are ortho, fam med, and PM&R. Anything else? Ortho is number one right now.
 
I could be wrong, but if you want to be on the sidelines I'd have to think fam med and PM&R would be much better options than ortho. Do surgeons ever man the sidelines? Sports med or not.

You'd be mostly wrong. Orthopaedic surgeons most certainly man the sidelines at events. Physiatrists, family/primary care physicans, and emergency physicians also cover the sidelines at major sporting events. But when you're watching an NFL game and a guy with a suit is examining an injured player it's usually an orthopod. You will see the other guys be the main doc and lesser sporting events like WNBA and MLS. Regardless of specialty, in general, the only guys that cover sidelines are sports medicine fellowship trained.

Primary care (via internal med) and emergency medicine are other avenues to sports med. Pediatrics is another avenue but WAY more limited than the adult pathways.
