Many of these programs provide significant exposure to and involvement in research, some with a great deal of autonomy for the student (though this depends on the project as well.) Though some will deny it, there's also a prestige element, especially if you're not from a prestigious undergrad; the idea that a top medical institution found your credentials respectable and was willing to devote resources to develop your potential as a scientist and scholar (as well as the fact that in going through such a program you will have effectively trained in such an institution) can add some degree of impact factor to one's application. For applicants from smaller schools or LAC's that lack substantial research opportunities, summer programs may be one's only chance to find meaningful research opportunities, and programs recognize this and many explicitly state that availability of research opportunities at an applicant's school factors into the decision-making process. Some applicants forego academic year research in favor of other activities and get research in during their summers. NickNaylor, known for his impressive application and success during his application cycle, had the entirety of his research experience through two summers of participation in a SURF program. In the end, like any other activity, much of the value is in what you get out of it. Some summer research participants publish, some don't, and of course some find it an incredibly rewarding experience and some don't. No matter where you do it, summer programs are a legitimate way to get exposed to research, and possibly have a rewarding and meaningful experience.How big is getting into one of these for applications to MD programs? They seem more competitive, after all, but don't include MCAT (for obvious reasons).

I did research at my school for 3 semesters and a summer, but I decided not to stay this summer. My PI sort of understood. He wasn't mad, but more disappointed that I wasn't as into the research as he was. I told him that I wanted to explore other fields and do something clinically related. (My research was on animal physiology and response.) At the end of the semester, though, we were lucky enough to "finish" the project. Our results basically sucked, and he said he was going to step back from it for a while, so I think he ended up being okay with it.
TL; DR: My PI understood, but he was disappointed.
^This. Mine was definitely disappointed that I wasn't staying to work over the summer and I can definitely tell he's sorta bummed that I'm really not as into the research as he is. However, I'm learning a lot of new techniques applicable to our lab work that no one at my university knows, so he's REALLY excited about that! He keeps emailing me to see what I've learned so far haha.
Overall, I think any good mentor will understand that internships like these are a great experience both for learning new things about research as well as networking, so while they'll be disappointed, they'll also be happy for you.

He just got tenure this past year (late fall/early spring I think?).
