I personally took a year off between second and third year, and it was great. I'm of course jealous that my former classmates are all matched, but I do feel like it was a good use of my time and didn't really take anything away from my performance in third year.

As far as schools that will allow it, I can only really speak about mine, WashU. We're obviously a very research-focused school and they encourage us to take time off for research. Probably 20 or so people take a year off after second or third year each year and the school has stipends for those students. I'd hope that most schools would be supportive of a year off, but some may not have funding for those students in which case you'd have to apply for your own funding or hope your PI will pay you or not have any income during that year, I guess.
