Which I assume is the correct title of EM/US people. What was fellowship like? Do you do mostly teaching residents? What stuff do you do that a graduate of a "strong" u/s program can't do? How many "plain ER" shifts do you do? Is u/s fellowship as much of an academic circlejerk as people imply?

I think the term is actually "Ultrasonographer".
 
You get a job at an academic facility that requires a fellowship. Or you run the US program for a community group or CMG. You probably do more US in your practice as well, but correlation is not causation. Those more likely to do more US are also more likely to do a fellowship.
