Can y'all cite current opinions on a Lovenox bridging question?
Consider:
74 y.o. female who is now anticoagulated with warfarin because of being heterozygous for Factor V Leiden.
She had a post op PE in '64 (with warfarin anticoagulation for six months after), a couple episodes of superficial phlebitis in the '70s, a TIA in '82, and unprovoked bilateral PEs in'85.
She is now scheduled for a spinal epidural steroid and will require warfarin suspension and eventual resumption.
Considering the prothrombotic effect of suspending and resuming warfarin and the inherent risks of Lovenox, should she be bridged with Lovenox?
