Plenty do it without the form.

At this late date, I doubt if it matters at all. Surely the volume of trannies is really low now...less chance for it to fall by the wayside.
 
It's fine. Your transcript should have your SSN on it so AMCAS can use that. My school sent out my transcript without the form, then called to tell me they found the form and were sending a second one with the form. I really don't think it mattered. Besides, if your AMCAS ID went on there also, you're golden. No worries.
