A patient of mine has extreme pain from POTS and the 12 mg/day of Suboxone just isn't cutting it anymore for him. He's also on Amitriptyline but would like to get off of this med. He tried to quit Amitriptyline but his nerve pain came back threefold.
I suggested switching to Methadone, as I think it work much better for chronic pain, and because I think it can replace both the Suboxone and the Amitriptyline for his pain. The only problem is my doctor's office limits me to prescribe only 30 mg/day of Methadone and, coming from 12 mg/day of Suboxone, I'm not sure if it will provide enough pain relief.
Do you think it would?
