Transitional years -- how do these work?

I'm a fresh-faced first year and recently I've heard a lot of these terms thrown around with the MS2s and 3s. Just to clarify, at the end of medical school & for advanced specialties, do we apply for the RESIDENCY or the PGY-1 TRANSITIONAL YEAR? Assuming it's the TY, when we do apply for residency? (During TY?)
