Does anyone know if having major depression disorder would disqualify you from being able to serve in the Uniformed Public Health Service Commissioned Corps?

It will be disqualifying during your initial medical screening. You can then request a waiver of the disqualification, at which point they will refer you to a military psychiatrist. The military psychiatrist will make read over all of your current psychiatrist's notes, will do his own evaluation, and will then make a recommendation to a board. The board will decide whether or not to issue a waiver based on the Psychiatrists recommendaion and how badly they need doctors that year.

In general if you've been functional off medication for awhile its not a big deal and you will probably be granted a waiver. If you still require medication it is a big deal and their response varies from year to year.
