What constitutes the need for an SMP or Post-Bacc

So i was just curious what the cutoff is that would require someone to do an SMP or Post-Bacc. Lets say that all the extracurriculars are there and a 508 MCAT but the cGPA is around a 3.5 and the sGPA is about a 3.4. Would this give rise to needing a post-bacc or should the applicant aim at lower tier MD's and DO's. If the applicant really wants MD would the post-bacc, masters or SMP do them well?
