where to find waiver form for SLOE

Just finished my 1st away and will be getting a SLOE. The clinical coordinator for the site said to send over an eamil requesting the SLOE along with the signed waiver to see it.

Where do I find this waiver? Ive googled the hell out of it and havent found anything. Its the weekend so I dont feel like contacting anyone in the program over this yet. Do I get this form from the program writting the SLOE?

If I understand what you are asking for, it is on the ERAS website. Log in, and find the tab for additional stuff (sorry, I don't have the site anymore, so I don't know specifically how to get to it) and get to the page about your letters of recommendation. Then you put in the evaluator for the SLOE, and there is a print out that you can send to the program so they know how to get the SLOE into your ERAS file.
