Who do I address a update letter/ LOI to?

I tried the search function but cant seem to navigate through all the update letter threads to find an answer.

Who do I address an update letter/ letter of interest to?

Should I start out with:
Dear <Medical school> Office of Admissions

Or should I address the Dean of admissions?

i would recommend you to e-mail the school of admissions letting them know that you will be sending an update letter/letter of intent. you should then write a formal letter addressed to the dean of admissions :)
 
At this school, you've had an interview and you're on hold, or you want an interview, or what?

No need to send them a message to say you're going to send a second message, that's annoying. And don't waste their time with a trivial update, most offices have more than enough work to do at this stage of the cycle.

It doesn't matter to whom your correspondence is addressed. At most schools, all incoming mail to the admissions office crosses the same admin desks and goes through the same hands. If you're lucky, it will make it into your file. It may not.
