So you talk to attendings, advisors, and other experts.

They all say one thing.

"Get into the best residency program possible".

Why is that?

Does reputation really that much down the road?

Deeply appreciate any input on this common advice.

They say that for a few reasons.

First, if you are thinking about moving on with your training for fellowship, generally the stronger the residency, the more options you have for fellowship.

Second, aside from reputation, at a "better" program you will often be exposed to the newest therapies. For internal medicine this doesn't mean all that much but at a large program they tend to get sent cool cases and you see more.

Third, the stronger the program, often the more autonomy so you tend to mature faster. This is not always the case though.

Now, it doesn't mean all that much if they are in similar tiers. So the difference between say Cornell and Columbia probably is not that much if you really liked cornell more than Columbia. It also doesn't mean much if you have family or economic concerns with a given area.
