Won't have CK in time... Apply without or late?

I am not taking CK until a few months so I won't have my score by Step 15.

I have passed CS and everything else is ready to go except for a letter of recommendation that I will not get until November (which is an important LOR)

Should I apply to programs with an incomplete application or should I wait until I have everything around late November/early December?
