At my outpatient clinic we rolled out Xofigo as a treatment option about 6 months ago...with changes in reimbursement we now lose money on injections..can't even break even.

Are other freestanding centers seeing this problem as well and how are you dealing with it?

It makes no sense to incentivize this treatment to be delivered only at hospitals.

Is this a hospital vs freestanding problem as well? I know when it comes to EBRT, hospital makes more. For much of the last decade, SRS with CK and GK paid much better in the hospital than freestanding (in fact, CK sometimes didn't pay at ALL). Med Oncs also get paid less to give drug in their clinics.

We haven't seen many xofigo pts since the new payment system started..... basically now, they charge you per unit and you bill per unit.

The drug interruption last year put a damper on things for us in terms of offering it to patients.... made us look bad when we had to stop treatments last year. Will have to look into this before we start getting into this again.

The entire deck is stacked against outpatient care in general, especially in oncology.
