Is every STEP 1 question a vignette ?

YES. If you want a better idea of how the questions are worded, there are many resources to look at:

-these are probably the most used resources, though there are probably many more as well.
 
Some questions will not require that you read the vignette. Example being a vignette about a STEMI patient who is started on warfarin prophylaxis. The vignette includes all his current and past medications and some other medically related crap. Then the question is "What is the most appropriate laboratory study to perform on patients taking Warfarin?". Don't need to touch the vignette to answer that question.

Wish it was that easy.
They purposely "insert" a useful piece of information within that "medically related crap" which changes the answer entirely.
 
Yes, they might have an inducer/inhibitor thrown in or even throw in pt with low levels of Vit.K and bleeding, which can change the answer entirely.

Through ~1500 qbank questions and several chapters of Robbins Review questions I haven't had that experience.

However, "Which lab should this patient have monitored?" is completely different than "Which lab should be monitored in patients taking warfarin?" Clearly the former is dependent on the vignette, whereas the latter is independent of the vignette.
