How are A- scored on the application ? How about B+ ?

No, it depends on your school. If your school has A-/B+ you are correct. If your school only does '+' grades or A/B, then a B+ is 3.5. AMCAS knows all the schools systems, and automatically works it out for you.
 
While most schools average -'s and +'s into their g.p.a., most medical schools do not. For instance, at Baylor Undergrad a B+ is a 3.5 g.p.a. At 7 of the 8 Texas medical schools, only Baylor Medical will count that extra .5 g.p.a. At every other school a B+ is no different than a B-.

Are you talking about while you are in medical school or while you're applying to medical school?

I don't know about while you're in med school, but when you use AMCAS, your +s and -s are definitely factored in. As decribed above, the relative values assigned are dependent on your undergrad university's grading system.

Which schools have + grades but no - grades?

Mine is private, but is transitioning to - grades this year. Too bad, that was a nice little boost to the GPA
