Are any of you in the NW states getting denials for anything lumbar facet-related? They gave us a huge list of crap that we have to send out any time someone is going in for lumbar facet procedures (pain logs, therapy notes, clinic notes, etc). If they don't have EVERYTHING its a denial of payment. For what we are paid, this is so f-ing ridiculous. I'm ready to go cash and tell all insurers to F.O.
