What are your goals?

I want to go into academic medicine and eventually do policy work and advocacy. Right now I'm really interested in EM because of the tie into disaster response. I don't see myself going for primary care instead of a speciality.

Im unsure what you mean. You want to go into academic medicine and do policy work, yet you do not want to go into primary care?

Not a whole lot of cardiothoracic surgeons doing policy work. Most of those folks are MD MPH with a primary care background.

Correct. It may be rare, but I've seen specialists like dermatologists and infectious disease physicians be involved with policy and advocacy.
 
If you're looking into Academic Medicine, then definitely go to the MD school. Pedigree matters in the academic world.
