Definitely go with waist down, but be prepared to prove it's you.

I don't think the MCAT people care much about what kind of photo it is, so long as they can use it to identify you, but since I think they request a "photograph", I wouldn't go with a printout. (although with a digicam, you can go to your drugstore and print it out onto film). Just do the normal passport style shot.

It really, really, really matter doesn't matter. I had a black and white printout on a shoddy printer and it was fine. As long as the picture is of you, you're fine.

Don't waste money on an actual photo. A clear picture on an ink jet is perfectly fine.

I am Danny Tanner, Bob Saget is really ficticious... Probably get in good with the proctors anyway when I sign some autographs for them, although I'm no Uncle Jessie, or Michelle for that matter

They don't want you making a black and white copy of some random picture that will be terrible quality. If they can tell who you are by the photo that's all the quality you need.

All the photograph is needed for is to check against your driver's license/passport (as well as you) to verify that you are the person on the admission ticket. As long as your picture accomplishes that, you're fine. Save the money for nice pics for your secondaries, where the pics presentation is a bit more important.
