you've posted in the forum for *pre*meds... who are applyng for MD admission into US MD schools. you might have better luck at one of the caribbean forum websites, from people who have already been through the process.
what was your freshman year gpa? you may still be able to salvage your gpa, but at this point, you'd probably have to do a post-bac (georgetown has one designed to boost gpa, i believe? it would take another year to do, like all the other special masters programs. and those people are right, you have slim chances of being accepted without further work)
you could apply to be a DO, instead of an MD. DO is functionally equivalent, you'd still be a doctor. admissions standards to DO school are much lower.
either postbac or caribbean, you face a huge upward battle. why not postpone this decision after another year of the "top 20" school? if you can get A's (maybe a B/B+ here and there) when you transfer back, then you'll probably want to consider a postbac and applying to American schools... and admissions committees will look favorably upon an 'increasing trend'. you should also retake the courses you flunked (what were they? intro bio? chem? -- med schools look at those courses)
