You're going to have to give us at least something to work with. Nobody here is going to construct your entire list from scratch. Get MSAR and start there. Your stats are stellar, so those won't really hold you back anywhere. Look for areas that you'd be willing to live, leave out out of state schools that don't accept many OOSers, then trim it to have an even balance of most selective to less selective schools.

Your final list might look like this: All Ohio schools, ~10-15 top 25 schools, ~10-15 25+ schools.
 
I really do think you have a great shot at a top-20. With your stats definitely apply to WashU and UChicago. You're going to have to come up with a strong reason why medicine is your calling since the PS is perhaps the single most important factor in your application. The best advice for the PS I can give is don't just stay that medicine is your calling, demonstrate it with specific examples/anecdotes.
