At least Hofstra and Einstein...
Even though they are private IS? Is there preference for IS at private schools?

Also, how do you think my chances are for other schools listed?

I'm inclined to disagree with my learned colleague, on the basis (admittedly narrow) that the SDNers I know who got into top schools have tons of volunteering, both clinical and non-clinical. I thus strongly recommend service to others less fortunate than yourself.

But you might try some in a cycle such as something like this:

Stanford or Vandy
Harvard or Yale
U Penn or U Chicago
Cornell or Columbia
NYU or Mt Sinai, etc.

Inquire into the school-specific thread and see if you can get a sense of what current students did in terms of ECs.

Agree 100% with the wise gyngyn that all your state schools should be on the list.

For volunteering, how much do you consider significant? Is it possible to get significant hours by June while working full time? And how much does a significant research experience make up for lack of volunteering?

Hundreds of hours. Some people have up to four digits worth. The higher up the pole you want the climb, the greater the competition.

It doesn't. You need to show off your altruism and humanism. This is a service profession, after all.

You have a good list and should receive several interviews. You could add SUNY Downstate and Mount Sinai.

Quick question: Does OP have an advantage at Cornell, NYU, Einstein, and Sinai because he's from NY?
 
I was summoned by the op.

If you want to do MD/PhD, focus on applying MD/PhD. Your application is strong for that. There is no reason to apply MD as a backup. You need to pick which one you want.
