Anybody else wish there was a 5 year EM/CCM combined residency instead of the 6 year model? Same length of training if you do a 3 year EM residency + CCM fellowship. Why not have a combined EM/CCM? Would be nice to match right into it instead of applying for fellowship. Just saying....

Yes is it a 2 year fellowship. But the only available direct match that includes CCM is EM/IM/CCM, which is 6 years. With the number of EM guys going into CCM it seems logical to offer a 5 year combined EM/CCM residency instead of the 3/4 year EM residency + CCM fellowship or the EM/IM/CCM combined. The EM/CCM model residency seems much more attractive than the EM/IM/CCM or EM + Fellowship model for those interested in CCM

Because none of the fellowships are primarily through ABEM. Hence, we can't.

I did EM + CCM. I see how it might be nice to have this option, but there’s definitely significant benefits to doing fellowship somewhere other than where you did residency. You learn a new way of doing a lot of things and double your network. Plus, life can change a lot during residency and your goals may change - you may not want to do CCM, you may want to do it at a different location, you may realize you prefer sicus to micus or cticus, etc.

There is a combined 6 year program because it leverages your early integration into the program for double credit in order to get people through faster. An EM/CC program wouldn't make sense unless they could chop it down to 4 years. Why would a program want to match you into a CC program 3 years in advance judging only on your performance as a medical student in hopes that you won't turn out to be a lackluster resident or lose interest in critical care?

Not a great idea. The 6 year programs aren’t either unless you want to stay in academics or something, you don’t need to have three boards. Coming from EM, we have a great setup too because you can still work ED shifts and do CC even in the community. Anesthesia and Surgery have a little bit harder of a time outside of academics. A better idea would be to have a 5 year critical care residency where you get a mix of Anesthesia/ER/IM/Surgery throughout your training but that won’t happen in the US.
