Here is a little Excel file I created to help estimate 2007 collections.
The numbers are easily changed to reflect your LMRP.

Those private averages you have up there are years old by New York standards. 200 % medicare for most stuff....

Are these in-office fees or professional fee at ASC?

Office based procedure fees.
ASC varies because there is set facility fee for MC, set pro-fee for MC, but for private insurers- it will depend on your contracts.

Please note I did not publish my numbers for the ASC or office as that would be anti-trust violations. I took an avergae of posted fees and reduced them to around the 25th percentile.
