academic promotion at VA

Can someone please explain to me what happens to academic physicians who are full-time employees at a VA but don't get promoted to say assistant or associate professor? If they are paid only by the VA, would they still get to keep their clinical job moving forward? Or is it like any other academic position where if you don't get promoted, you have to pack your bags and find another job elsewhere?
