additional forms for letters of recommendations to fill out?

i submitted my primary fairly recently...haven't received any secondaries...but just want to know if the LORs have any additional forms or evaluations to fill out for any of the following schools...

or is it the simple letter? (im canadian)

Albert Einstein
Brown
Dartmouth
Howard
Jefferson
John Hopkins
Mayo
Northwestern
Rosalind Franklin
University of Pittsburgh
Wake Forest
Wayne State
