Would it be a red flag on my app if my shadowing was something like this: 20 hours psychiatrist, 30 hours emergency medicine, 30 hours neurologist? It's not that I'm not interested in primary care, I just haven't found anyone in that area that will let me shadow.

Psychiatrists are considered to be Primary Care Providers. And Neurologists often provide longitudinal care for patients with chronic problems, even assuming overall care for those with some conditions. So I think you're fine as you are.
