I'm an MS4 at a mid tier (our school doesn't participate in ranking, but i think it would fall somewhere at the bottom of the top 40-50 schools). I had my obligatory meeting with our dean today, I told him that I would like to go back home to California/Pacific north west for residency. He basically told me I don't have much of a chance at any of the "top programs" out there. I'm in the top half of my class. Honored Medicine, high passed every other rotation. Also honored my sub-i. I got a 246 on Step 1. taking CK in less than two weeks. Have done some good research, and some solid extracurricular public health projects.

He was telling me that even OHSU, and UCSD might be a reach. Is that true? Should I not even apply to the more competitive programs there? It seems like I have to apply much more broadly than I originally thought. Is there anything I can do at this point to help me, or just have to apply and hope for the best?
