Butrans, the drug denied in the first anecdote, is an addictive opioid, not an opioid alternative. It's the expensive skin patch form of buprenorphine. I use buprenorphine for treating my addiction patients. The generic sublingual tablets and Suboxone sublingual film are also used off label for pain.
