Do you believe that we all start on equal footing?
Do you believe that you are in your position because of your efforts alone?...could everyone else with equal mental and physical abilities be in your position to determine who makes "smart" decisions and therefore deserves medical care?

No, we do not all start on equal footing. That is also irrelevant, as my argument was that socialized healthcare isn't a "unique social good." Your argument is only relevant if you believe that equality of cirumstance (notice I didn't say treatment under the law) is a social good. I don't believe that it is. This is the underlying tenent that eventually destroys all large scale nationalized economic structures. We are all born with certain attributes and weaknesses. We have certain social advantages and disadvantages. We make the best of what we have. This doesn't mean that I owe something to those born with less than me. It likewise doesn't mean that I am owed anything by those born with more. Medical care isn't deserved. It is something that is created by the combined efforts of many individuals. If every doctor went on strike tomorrow, there would be no healthcare.
By the way, I know more than one person who landed here in Miami on a makeshift boat and now has a seven figure bank account. I will not buy the argument that a strongly motivated person can't make enough in modern America.
