Few (no?) programs would allow you to come in as a PGY-3 given the learning curve with learning the intricacies of the program, hospitals and EMRs. Plus you'd be supervising junior residents at a program you know nothing about. At the very least they'd likely make you complete 6 months as a PGY-2 if not repeat the whole year.
Positions open up all the time.
MVH in Las Vegas only hired 18 PGY2s last year, and this year they hired a new PGY3 who had completed two years elsewhere.
They only have 19 PGY3s, and are allowed for 20 total.
Completely not correct that positions open up all the time. There are never PGY-3 medicine postings on the AMA vacancy site, and FindA Resident has postings from 2005.
