I want to one day practice pediatrics in one of the states that a rotating internship is required... Can I apply to an allopathic program (where I want to do peds) at the same time I apply for my rotating internship at a different place... all during fourth year?

DO NOT do that. Just apply to peds residency ACGME then petition the AOA to accredit your 1st year to allow you to get a license in those states. Happens all the time and the hoops are pretty easy to jump through. When I was a fourth year I spoke with someone who did exactly that (DO Peds resident at Cardinal Glennon) and she had no problem at all getting it approved.

This was the same exact question that I had! When do you petition the AOA? During your first year? After completion of the program? 😕
