Army Handbook Question

Hey guys need some help. I am out on ADT and need the following information (which is in the HPSP handbook convienently left at my home).

I need the address for the Student Management Office and the fax number if you have that as well.

You may locate the address to the student management office in your policy handbook Chapter 1 Letter D.
