Hi there,

Just wondering how successful you have been in switching your legacy opioid patients over to butrans.

The butrans website states that if the patient is taking over 80 MME per day, butrans may not work, and they have no recommendations for butrans dose (if I read it correctly).

Thanks for sharing your experience.

You are going to have to wean them to at most 60med to have a chance in my experience

Exactly. Most patients don't need 80 anyway.

I would wean to at least 60, likely 40 before converting.

I do wish we had available to us in the US, the higher butrans doses that are available in Europe.

Legacy opioid patients hate butrans. But that shouldnt stop you...

Yes, don't try to be cute by prescribing multiple patches or a 5 day schedule. It will never be allowed.

I have patients on 10mcg they change on mondays and 5mcg they change in Thursdays. Effectively 15mcg and less complaining of patch lasting 5-6 days.
