What would be the proper course of action to take regarding a misfill for a C-II (For instance - Rx written for 75 mcg patch but 50 mcg dispensed)?

More specifically what forms would have to be filled out to fully comply with the DEA?

Any help is appreciated.

I beleive it is Dea Form 106 - Theft or Loss of Controlled Substances

And you'd check other and fill out the reason for the loss. Now it also states it has to be a "significant" loss and I don't quite remember what the definition of significant is, but I beleive it is more than you'd think (like 1% of all sales for the year).
 
I think it's 5% of your yearly volume that you are allowed to be off. I always thought of that form as for actually thefts and losses, not really for documentable errors. But, I guess you could use it for a mis-fill.

I would hope that the error is also being corrected if the medication is currently in use.
