California licensure application procedure

My friend is a foreigned dentist. She passed exams (RT and WREB) required for Cali licensure. In the form" Application for licensure to practice dentistry (WREB) after successful completion of the restorative techinque exam" - item 10 - the question is "Have you ever been issued a license in any state or country?" If yes, certification of license must be submitted for each State/Country.

She wish she could click "no" to that question but she couldn't. When she applied for RT exam 3 years ago, she did submit the license for her country. Now, it is really hard to fly back to her country to ask licensing agency certify that form. Any of you meet such a problem and what did you do? Can my friend submit the application without submitting certification of license in her country.

may be she can send a copy of her old licence saying that she has not renewed it since last few years?
they may not be asking for a current licence.
they ask HAVE YOU EVER BEEN issued?
so it means that she ahs been issued a licence but she is no longer staying htere so she has not renewed it.
