Can you do your residency at one place and a fellowship somewhere else?

Sure. Just like with medical schools, some fellowships PREFER their own residents and some prefer outside blood. You are by no means required to do your fellowship at the same place you do residency - it just makes it easier because everyone knows you and can more easily evaluate you (no guarantees - another outside applicant might be more appealing - the deadly sin of being known "too well" - warts and all!)
