I really want to end up in the Bay Area when I am a practicing physician due to family. What is the best way to "secure" a spot in a certain geography? Would it be better to go to a community program in the bay area (i.e. kaisers, cpmc) or would it be wiser to go to a university program like U of Iowa IM?? More impressive to the hospital? or better to hire someone within the system? (planning for the 2012 match!)

What do hospitals/groups hiring look for? Connections to the Bay Area?.... or more Where did they do their residency...ooohh a nice academic program like U of Iowa? or How is this person going to fit in to our group/hospital system?

Is it easier as a specialist (cards/gi/hemeonc/rheum) to be able to secure a spot in a certain geography? More options if one specializes?

Do you contact physician reps from the area?

Sorry for the naive questions...just wondering what an internists market looks like and opinions about where it is going in the future...

As an internist or sub-specialist prospects look good across the board for the near furture as most of us will be finishing our training. You will obviously, and a generalization, make LESS money in "more desireable" markets, such as Northern California, but you should be able to find a job. There are plenty of recruiters and they will spam the hell out of you as much as possible as you end your training.
