Hello, I was wondering if anyone can give any advice how to grow as a pharmacist on the PBM or industry side. A little background of me is I worked in retail for five years as a pharmacist. Then I worked at a specialty pharmacy for three years as a clinical pharmacist. I am currently working for a PBM these past two years as a prior authorization pharmacist. I’m trying to advance my career, but it has been difficult. I have applied on and off to numerous positions this past year with no success (some examples-account manager for client, formulary management, program manager, supervisor, drug information, medical information manager). Any insight would be appreciated. Thanks!

If you’re not a natural salesperson, you need to become one. And what you are trying to sell is yourself, not necessarily a product. And most of the time, to climb the corporate ladder, you have to change companies. There’s very rarely (as I can count the number on one hand) a person who successfully stays at an industrial practice for more than 20 years. And none of them advanced to what they were capable of.

I don’t have advice in particular on growing within a PBM, but I’m also interested in this as well . I also work for a PBM but doing patient consultations . I’ve been working here a little under 3 years by now and I enjoy my role but would also like to experience what else is out there . All those titles about formulary management, account manager for clients, etc some of those roles when I read the job descriptions they are over my head and I don’t feel I qualify for any of them . I worked retail for 9 years and since early 2022 in my current role . My job is mostly phone based and even though I like it and good at it , I’d like to eventually get off the phones and venture into a new role . Maybe others with even more PBM experience can chip in . How do you like PA and what did you think of specialty ? I hear specialty can be nice and also chill .

I understand where you’re coming from as I felt stuck in my roles. Currently looking at moving into clinical programs within my company. Fingers crossed I’m able to move in the next few weeks.

Keep in mind it is a difficult market right now and without the ability to sell yourself, it will unfortunately become hard. You’ll also have to network as we recently filled some roles and every single one was a referral.

Keep in mind it is a difficult market right now and without the ability to sell yourself, it will unfortunately become hard. You’ll also have to network as we recently filled some roles and every single one was a referral.
Personally, I’m going to start looking for other roles next year at the 3 year mark after I collect my bonus and get my pay raise . My job is what most here would call a “unicorn” job because it is a walk in the park to me compared to retail, it’s chill, and all I do is counsel patients on inbound calls regarding their prescriptions and then document the call . Simple as that . I feel absolutely 0 stress . And I will even have 28 days of PTO next year . How many pharmacists have that much PTO within 3 years ? That is European level vacation time /PTO. I will look to see within my PBM just to see if there are higher paying roles and may look externally as well . But I’ll be very cautious - I won’t be leaving for a $2-$3 increase in salary . For me to leave it’d have to be at least $5-$10 an hour increase per hour . I’ll dust off my resume and try to tailor it to the position I apply for but I have to admit most of the pharmacist positions I’ve seen within PBMs require extensive knowledge of managed care, formulary design, client facing , Medicare , etc and when I read those job descriptions it almost doesn’t even sound like a pharmacist position and I don’t bother because I don’t qualify and wouldn’t know a single thing about the job duties . It seems to be the only role I would be capable of performing for a PBM with no MC experience etc is prior authorization . So, we’ll see . I’ll apply everywhere next year but if I realize I’m better off in my current role , I’ll stay . I’m not moving anywhere for $3 more and only 14 days PTO either .

The number stated sounds like Optum after 5 years (from my memory at my time there). I remember it was 23 pto days (28 after 5 years) plus federal holidays. I don’t remember having separate sick days at Optum.

At my current PBM I get 25 days PTO, 14 sick days plus federal holidays.

Not Optum . It’s express scripts/cigna .

Honestly it’s still not a bad deal. 28 days is basically a little over 5 weeks plus fed days. So like 7 weeks off including fed days (28 PTO and stated 8 fed days)

I know . That’s what I’m saying. That is European levels of vacation time “holiday” as they say . Most people in the US get 2 mediocre weeks of PTO and if lucky get up to 3-4 weeks after 15-20 years with their employers . PBMs are very generous when it comes to PTO.

Wow 25 Vacation days + 14 Sick days + 8 (?) Federal holidays = 47 PTO days...that is very generous! 👍

It is indeed! Likely why we have almost negligible turnover in our department and why every new position is basically filled with a referral at this point.
